Ideally this would be obsolete with #1889790: [Meta] Allow modules to register links for menus, breadcrumbs and tabs (if not with hook_menu), but in the meantime, this is a nasty bug.
If in your hook_menu(), you give a route_name to a menu item that is a MENU_DEFAULT_LOCAL_TASK, all of your menu structure will be broken.
Sure, it's a bit of hand-holding, but that's what happens when you have two systems at the same time.
